Once the questionnaire has been filled out for an item and any requested documents have been uploaded, the process is complete for that item. If any modifications are required an email will be sent to you by the Costco team.
You have 14 calendar days to complete the questionnaire before escalation to Costco. The requested document(s) are required to be completed prior to importation into the US.
Requests for extensions beyond 14 days should only be made in extreme circumstances and need to be directed to the Costco QA team.
No, an account does not need to be created. Simply follow the instructions in the email and click on the “Costco CoC Management” link or links to complete the process and submit any required documents.
The “Costco CoC Management” link gives you the option to add products proactively:
If a questionnaire is being requested, this means Costco does not have compliance on record for the requested item number. Please kindly complete the questionnaire and upload any requested document(s) as required through the “Costco CoC Management” link.

Yes. Compliance is required for items that have been sold at Costco at any time.
